🔸 Upload the most recent valid Proof of Income for your parent(s) or guardian (dated 2026, or 2025 for the ITR). If no Proof of Income is available, you may submit a notarized Affidavit of Income Source indicating the monthly or annual gross income of the parent(s), family member(s), or guardian who contribute to the household income or support your education.

🔸Kindly make sure that all other required documents are submitted on or before the deadline to ensure that your application can be properly reviewed and considered for processing.

Qualifications

Primary Qualifications

These qualifications are automatically validated based on student portfolios created

Overall GWA must be better or equal to: 2.0000
Parent Annual Income must be equal or lower than: Php 500,000
University must be within the ff:
Courses must be within the ff:
Year Level Standing must be within the ff:
Must have no grade of "5.0", "DRP", nor unremoved grade of "4.0" or "INC" for ANY semester
Must not be taking secondary bachelor's degree

Secondary Qualifications

These qualifications are subjected to moderator review

Must be a senior B.S. Mathematics student enrolled in UP Diliman.
Has completed at least 45 units in Mathematics with an average of at least “2.0” and is enrolled in at least 15 units at the time of award.
Does not have a grade of “5.0” or unremoved grade of “4.0” or “Inc.” in the immediately preceding semester.
Is of good moral character as evidenced by a certificate issued by any of the following: Office of Student Ethics, Dean of the College, a faculty member of the University.
