🔸Upload the latest valid Proof of Income of your parents or guardians (dated 2025, or 2024 for ITR). If no Proof of Income is available, you may submit a Notarized Affidavit of Income Source indicating the monthly or annual gross income of the parent, family member, or guardian contributing to the household income or supporting your education.
🔸Kindly make sure that all other required documents are submitted on or before the deadline to ensure that your application can be properly reviewed and considered for processing.

- Must have no grade of "5.0", "DRP", nor unremoved grade of "4.0" or "INC" for ANY semester
- Must not be taking secondary bachelor's degree
Secondary Qualifications
These qualifications are subjected to moderator review
- Must not have been the subject of any disciplinary action worse than a five-day class suspension.
- Must be a Filipino citizen
- Must have carried a load of at least 15 units with no grade of “5.00” or unremoved grade of “4.00” or “inc.” in the semester immediately preceding the application.
- Must not be a recipient of any financial grant or scholarship.
