If no Proof of Income is available, you may submit a notarized Affidavit of Income Source indicating the monthly or annual gross income of the parent(s), family member(s), or guardian who contribute to the household income or support your education.

- Parent Annual Income must be equal or lower than: Php 500,000

3rd Year Undergrad
- Must have no grade of "5.0", "DRP", nor unremoved grade of "4.0" or "INC" for ANY semester
- Must not be taking secondary bachelor's degree
Secondary Qualifications
These qualifications are subjected to moderator review
- Must have an average of at least "2.50" or better and must have carried a load of at least 15 units with no grade of "5.00" or unremoved "4.00 or "INC".
- Must not be a recipient of any other scholarship grant.
- Must not have been held liable for any disciplinary action worse than a five-day class suspension.
